Social Worker

At the highest level of, OpusCare of Florida Employees must a) be committed to hospice philosophy and mission and b) Provide outstanding customer service to customers. This position will provide support and medical social services to terminally ill patients and their families in accordance with an appropriate plan of care. The Social Worker will assume responsibility for developing, implementing, and managing psycho-social services provided to hospice patients and families in the home care or residential setting. Must have a Master's Degree in Social Work from an accredited program and be available to work across Miami-Dade county based on company needs.

Community Outreach Representative

At the highest level of, OpusCare of Florida Employees must a) be committed to hospice philosophy and mission and b) Provide outstanding customer service to both internal and external networks. The Outreach Representative is a professional consultant who is responsible for developing and maintaining relationships with physicians, physician office staff, social workers, and case managers, to increase referral activity and admissions into OpusCare of Florida.

Texas

Registered Nurse

At the highest level of OpusCare of Texas employees must a) be committed to hospice philosophy and mission and b) Provide outstanding customer service to both internal and external customers.  As a member of the Interdisciplinary Team, the "Field Nurse" works under the direction of the "Director of Clinical Management" and/or in her/his absence the "Nursing Supervisor," and the patient's/Hospice physician in the provision of skilled nursing care & services, assessment, planning, and care to maximize the comfort and health of patients and families.  Responsible for providing nursing care and services, assessing and identifying patient/family needs, and providing skilled care and supportive services in accordance with the patient's physician's orders and plan of care and OpusCare's policies and procedures. Demonstrate nursing practice skills and the ability to assess and respond to patients' and families' needs in varied settings and the ability to execute a supportive, sensitive approach to patient/family/personal caregiver (PCG) or significant other (S/O) needs. Available to work weekends.

Corpus Christi

Registered Nurse
